Penetrating Oculo-Orbital Wound Caused by a Machete with Rupture of the Eyeball: A Case Report from Bouaké

RESUME
Les traumatismes oculo-orbitaires par arme blanche sont rares mais graves, pouvant entraîner une perte définitive de la vision. Nous rapportons un cas de rupture complète du globe oculaire gauche associée à des fractures orbitaires chez un patient de 17 ans victime d'une agression à la machette. Le patient a été admis aux urgences une heure après une agression par machette ayant occasionné un traumatisme crânio-facial direct. L'examen clinique objectivait une plaie ouverte avec un important œdème péri-orbitaire gauche, un score de Glasgow à 15/15 et une absence de perception lumineuse du côté atteint. La tomodensitométrie cranio-encéphalique, réalisée 9 heures après le traumatisme, a mis en évidence une fracture des parois supérieure et inférieure de l'orbite gauche, une destruction complète du globe oculaire, un emphysème sous-cutané et intra-orbitaire, ainsi qu'un hémosinus maxillaire homolatéral. Aucune lésion intracrânienne n'a été identifiée. Le diagnostic de traumatisme oculo-orbitaire ouvert majeur avec destruction complète du globe oculaire et fractures orbitaires a été retenu. Le pronostic visuel était définitivement compromis. Ce cas illustre la gravité des traumatismes oculaires par arme blanche et souligne l'importance d'une imagerie précoce (TDM) pour guider la prise en charge multidisciplinaire. La prévention des violences par armes blanches et l'accès rapide à des soins spécialisés sont essentiels pour limiter les séquelles fonctionnelles et esthétiques.
ABSTRACT
Blade-related eye and orbital injuries are rare but serious, and can lead to permanent loss of vision. We report a case of complete rupture of the left eyeball associated with orbital fractures in a 17-year-old patient who was the victim of an attack with a machete. The patient was admitted to A&E one hour after being attacked with a machete, resulting in direct craniofacial trauma. Clinical examination revealed an open wound with significant left periorbital oedema, a Glasgow Coma Scale score of 15/15 and an absence of light perception on the affected side. A cranial CT scan, performed 9 hours after the injury, revealed fractures of the upper and lower walls of the left orbit, complete destruction of the eyeball, subcutaneous and intraorbital emphysema, and ipsilateral maxillary sinus haemorrhage. No intracranial lesions were identified. The diagnosis of major open oculo-orbital trauma with complete destruction of the eyeball and orbital fractures was established. The visual prognosis was definitively compromised. This case illustrates the severity of eye injuries caused by bladed weapons and highlights the importance of early imaging (CT) to guide multidisciplinary management. Preventing stabbings and ensuring rapid access to specialist care are essential to minimise functional and cosmetic sequelae.
