Intestinal Intussusception in an Adult Woman in Bozoum, Central African Republic

Acute intussusception is a rare cause of bowel obstruction in adults, often secondary to an underlying organic lesion. We report a case of ileo-caecal intussusception in a 64-year-old female patient in a resource-limited setting in the Bozoum district (Central African Republic). The patient was a 64-year-old multiparous woman who presented with abdominal pain and postprandial vomiting that had been present for five days. Clinical examination revealed tenderness in the right iliac fossa with peritoneal signs (positive Rovsing’s and Blumberg’s signs). Given the suspicion of acute appendicitis and the unavailability of imaging, a laparotomy was performed. Surgical exploration revealed an ileo-caecal intussusception with necrosis, which was successfully reduced under the telephone guidance of a remote surgeon, followed by resection and anastomosis. The post-operative course was uneventful, with the patient discharged home on day 8 and showing a favourable clinical recovery prior to her return to her village. This case illustrates the challenges of the surgical management of abdominal emergencies in an isolated setting and highlights the value of telemedicine (WhatsApp) for remote assistance from specialists. Better access to imaging and histopathology is required to improve the management of these conditions.
