Abstract

RÉSUMÉ
Introduction. Le cancer de l'estomac connaît une progression marquée de son incidence depuis une décennie, avec un diagnostic trop souvent posé à des stades avancés ou métastatiques. En situation métastatique, la prise en charge repose sur une chimiothérapie palliative, parfois associée à une thérapie ciblée anti-HER2 (trastuzumab). Cette étude a rapporté l'expérience clinique du protocole FOLFOX, avec ou sans trastuzumab, dans le traitement du cancer gastrique métastatique. Méthodes. Étude rétrospective descriptive menée au service de cancérologie du CHU de Treichville sur 4 ans (janvier 2019 à décembre 2022). Nous avons inclus tous les patients présentant un cancer de l'estomac métastatique confirmé par l'histologie ayant reçu au moins six cures de FOLFOX (oxaliplatine, 5-fluoro-uracile, acide folinique), avec ou sans trastuzumab. Résultats. Quarante-six patients ont été inclus (âge moyen 53 ans ; sex-ratio H/F 2,5). La douleur épigastrique était le principal signe clinique (65,2 %). L'adénocarcinome moyennement différencié représentait 39,2 % des types histologiques, et une surexpression de HER2 a été observée chez 19 % des patients. Les sites métastatiques prédominants étaient le péritoine (35 %) et le foie (33 %). La toxicité hématologique était la plus fréquente (50 %). En termes d'efficacité, une stabilisation de la maladie a été notée dans 39 % des cas et une réponse partielle dans 26 %. La survie globale à 12 mois s'élevait à 65 %, avec un taux de décès de 32 % sur la période d'étude. Conclusion. Le protocole FOLFOX démontre une activité thérapeutique certaine dans le traitement du cancer gastrique métastatique. L'intégration systématique des thérapies ciblées anti-HER2 semble indispensable pour optimiser la survie globale et la qualité de vie des patients. Ces résultats ouvrent la voie à la nécessité d'explorer de nouvelles stratégies, comme l'immunothérapie, à travers des études prospectives de plus grande envergure.
ABSTRACT
Introduction. Gastric cancer has shown a marked increase in incidence over the past decade, with diagnosis often made at advanced or metastatic stages. In the metastatic setting, management relies on palliative chemotherapy, sometimes combined with anti-HER2 targeted therapy (trastuzumab). This study reported the clinical experience with the FOLFOX regimen, with or without trastuzumab, in the treatment of metastatic gastric cancer. Methods. We conducted a retrospective descriptive study at the oncology department of Treichville University Hospital, Ivory Coast, over 4 years (January 2019 – December 2022). We included all patients with histologically confirmed metastatic gastric cancer who received at least six cycles of FOLFOX chemotherapy (oxaliplatin, 5-fluorouracil, folinic acid), with or without trastuzumab. Results. Forty-six patients were included (mean age 53 years; sex ratio M/F 2.5). Epigastric pain was the predominant symptom (65.2%). Moderately differentiated adenocarcinoma accounted for 39.2% of histological types, and HER2 overexpression was observed in 19% of patients. The predominant metastatic sites were the peritoneum (35%) and liver (33%). Haematological toxicity was the most frequent (50%). In terms of efficacy, disease stabilisation was noted in 39% of cases and partial response in 26%. Overall survival at 12 months was 65%, with a death rate of 32% during the study period. Conclusion. The FOLFOX regimen demonstrates definite therapeutic activity in metastatic gastric cancer. However, systematic integration of anti-HER2 targeted therapies appears essential to optimise overall survival and quality of life. These findings open the way for exploring new strategies, such as immunotherapy, through larger prospective studies.
