Giant Lipoma of the Right Spermatic Cord Mimicking an Inguinal Hernia in an Ivorian Patient: A Case Report

Spermatic cord lipoma is a rare benign tumor whose clinical presentation frequently mimics that of an inguinal hernia, posing a diagnostic challenge. Giant forms, defined by a size exceeding 10 cm or a weight exceeding 100 g, are exceptional. We report the case of a 44-year-old man operated for a right inguinal swelling of two-year duration, in whom surgical exploration revealed a well-encapsulated giant lipoma measuring 12×8×5 cm and weighing 180 g, with no associated hernial sac. Complete excision with mesh hernioplasty was performed. Histopathological examination confirmed the benign nature of the lesion. Postoperative recovery was uneventful, and three-month follow-up showed no recurrence. This case illustrates the diagnostic pitfalls of this entity and the need for systematic cord exploration during inguinal surgery in sub-Saharan Africa.
