Sociodemographic, Clinical and Judicial Characteristics of Mentally Ill Prisoners in Casamance, Senegal: A 6‑Year Retrospective Study

Introduction. Malgré les alertes de l'OMS sur la forte prévalence des troubles psychiatriques en prison, la recherche scientifique sur la santé mentale des détenus reste insuffisante face au surpeuplement carcéral mondial. L’objectif de l’étude était de décrire les caractéristiques sociodémographiques, cliniques et judiciaires des détenus malades mentaux de la Casamance suivis en milieux psychiatrique. Méthodes. Nous avons réalisé une étude rétrospective, descriptive et analytique, portant sur l'ensemble des détenus présentant des troubles psychiatriques et référés à l’hôpital psychiatrique de Ziguinchor, l’unique structure de santé mentale de la Casamance, sur une période de 6 ans allant du 1er janvier 2019 au 31 décembre 2024. Les données ont été recueillies à partir des dossiers médicaux et des registres de détention. Résultats. Cette étude portant sur 46 détenus, principalement des hommes célibataires d'un âge moyen de 35 ans, révèle une lourde charge psychiatrique : plus de la moitié présentent des antécédents psychiatriques, la schizophrénie est le diagnostic prédominant (37%), et le taux de consommation de substances psychoactives (tabac et cannabis) s'élève à 52,2%. Sur le plan judiciaire, la majorité est condamnée, le vol étant le principal motif d'incarcération, tandis qu'aucune corrélation significative n'a été établie entre l'usage de substances et les facteurs sociodémographiques ou judiciaires. Conclusion. Le profil du détenu suivi en psychiatrie — un homme jeune, célibataire et peu instruit, souffrant souvent de troubles psychotiques et d'addictions — met en évidence l'urgence d'un dépistage systématique dès l'incarcération et d'une collaboration accrue entre la justice et la santé pour protéger cette population vulnérable.
ABSTRACT
Introduction. Despite WHO warnings regarding the high prevalence of psychiatric disorders in prisons, scientific research on inmates' mental health remains insufficient in the face of global prison overcrowding; thus, this study aimed to describe the sociodemographic, clinical, and judicial characteristics of mentally ill detainees in the Casamance region followed in a psychiatric setting. Method. We conducted a retrospective, descriptive, and analytical study including all inmates with psychiatric disorders referred to the Ziguinchor psychiatric hospital—the sole mental health facility in Casamance—over a 6-year period from January 1, 2019, to December 31, 2024, with data collected from medical records and detention registers. Results. This study of 46 inmates, predominantly single men with a mean age of 35, reveals a heavy psychiatric burden: more than half had personal psychiatric histories, schizophrenia was the predominant diagnosis (37%), and the rate of psychoactive substance use (tobacco and cannabis) reached 52.2%. On the judicial level, the majority were convicted, with theft being the primary reason for incarceration, while no significant correlation was found between substance use and sociodemographic or judicial variables. Conclusion. The profile of the detainee followed in psychiatry—a young, single, and poorly educated man, frequently suffering from psychotic disorders and addictions—highlights the urgent need for systematic screening upon incarceration and enhanced collaboration between the justice and healthcare systems to protect this vulnerable population.
