Abstract

Intracranial abscesses of dental origin are very rare. This study reports two cases of intracranial empyema of dental origin to describe the diagnostic features and treatment approaches. The first case involved a 36-year-old patient who was admitted for altered consciousness and sudden left hemiplegia. Examination revealed poor general condition and a fever of 39.2°C. A brain CT scan revealed a left parietal extradural empyema. A dental examination revealed chronic suppurative mandibular osteitis involving tooth 47. Drainage of the collections and tooth extraction, combined with antibiotic therapy, led to a favorable outcome. We had another 29-year-old female patient who was hospitalized for acute cellulitis with a collection in the right lower genial region of dental origin (tooth 47). On the second day, she presented with tonic-clonic seizures. A brain CT scan revealed a left fronto-parietal subdural empyema. Drainage and tooth extraction, combined with antibiotic therapy, led to a favorable outcome. Intracranial suppurations are life-threatening conditions that can complicate a dental infection. Prevention and good oral hygiene are imperative.
