Abstract

RÉSUMÉ
Introduction. La neutropénie fébrile (NF) chimio-induite est une complication fréquente et grave des traitements anticancéreux, dont la prise en charge rapide et codifiée conditionne le pronostic. En Côte d’Ivoire, les données sur cette complication sont limitées. Cette étude a décrit les caractéristiques cliniques, thérapeutiques et évolutives de la NF chez les patients atteints de cancers solides au CHU de Treichville. Méthodes. Étude transversale rétrospective menée de janvier 2019 à décembre 2024, incluant tous les patients âgés de 18 ans et plus, atteints de cancers solides confirmés, ayant présenté au moins un épisode de NF chimio-induite. Résultats. Deux cent dix‑huit épisodes de NF ont été colligés (âge moyen 51,8 ans, sex‑ratio H/F 0,4). Le cancer du sein prédominait (41,3 %). Les protocoles les plus impliqués étaient les sels de platine (54,1 %) et les anthracyclines (46,8 %). Un faible risque de gravité selon le score MASCC était noté dans 78,9 % des cas, facteur associé à une meilleure évolution. Un foyer infectieux cliniquement documenté était présent chez 68,8 % des patients (ORL 22,9 %). Les patients ont reçu en moyenne trois injections de facteurs de croissance (G‑CSF). L’antibiothérapie probabiliste initiale était dominée par la ceftriaxone (59,7 %), souvent associée au métronidazole (39,4 %). L’évolution a été favorable dans 96,3 % des cas, avec une durée moyenne de prise en charge d’environ 7 jours. Une évolution défavorable (décès) a été observée dans 3,7 % des cas. Conclusion. La neutropénie fébrile chimio-induite est une complication fréquente mais de pronostic favorable dans cette cohorte ivoirienne, grâce à une prise en charge adaptée. Le score MASCC semble utile pour stratifier le risque.
ABSTRACT
Introduction. Chemotherapy‑induced febrile neutropenia (FN) is a common and serious complication of anticancer treatment, requiring prompt and standardised management. In Ivory Coast, data on this complication are limited. This study described the clinical, therapeutic and outcome characteristics of FN in patients with solid cancers at Treichville University Hospital. Methods. We conducted a retrospective cross‑sectional study from January 2019 to December 2024, including all patients aged ≥18 years with histologically confirmed solid cancer who experienced at least one episode of chemotherapy‑induced FN. Results. A total of 218 FN episodes were recorded (mean age 51.8 years, M/F sex ratio 0.4). Breast cancer predominated (41.3%). The most frequently implicated chemotherapy regimens were platinum salts (54.1%) and anthracyclines (46.8%). A low risk of severity according to the MASCC score was noted in 78.9% of cases, a factor associated with better outcome. A clinically documented infection focus was present in 68.8% of patients (ENT 22.9%). Patients received a mean of three growth factor (G‑CSF) injections. Initial empirical antibiotic therapy was dominated by ceftriaxone (59.7%), often combined with metronidazole (39.4%). Outcome was favourable in 96.3% of cases, with a mean management duration of approximately 7 days. Unfavourable outcome (death) occurred in 3.7% of cases. Conclusion. Chemotherapy‑induced febrile neutropenia is a frequent complication but has a favourable prognosis in this Ivorian cohort, thanks to appropriate management. The MASCC score appears useful for risk stratification.
