Abstract

Introduction. Femoral neck fractures are a significant public health concern, carrying life‑threatening risks in the elderly and the potential for impaired hip function in younger patients. Surgical management is the mainstay of treatment, with early weight‑bearing as a primary goal. This study aimed to evaluate the functional outcomes of patients after femoral neck screw fixation. Methods. We conducted a retrospective cross‑sectional observational study following STROBE guidelines, including patients operated on between January 2015 and December 2024. We assessed fractures using the Garden classification and functional outcomes using the Postel‑Merle d'Aubigné (PMA) score. We performed statistical analysis using Epi Info 7.2.2.6. Results. Twenty‑three patients were included. Femoral neck fractures occurred mainly in young adult males (n=14; sex ratio 1.5) following road traffic accidents. At a median follow‑up of 40 months (range 12‑84), functional outcomes were excellent or good in 75% of stable fractures (Garden I‑II), and fair or poor in 73.3% of unstable fractures (Garden III‑IV). Outcomes were significantly better in stable fractures (p=0.03). Complications were observed only after unstable fractures (p=0.04), including avascular necrosis of the femoral head (n=4) and non‑union of the femoral neck (n=2). Conclusion. Triple screw fixation provides better functional outcomes in stable femoral neck fractures. It remains a viable option for unstable fractures in resource‑limited settings while awaiting potential arthroplasty.
