Local-Anesthesia Excision of a Giant Gingival Fibroma in a Senegalese Woman: a Case Report

BSTRACT
Les fibromes gingivaux sont des lésions bénignes du tissu conjonctif, le plus souvent considérées comme des hyperplasies réactionnelles secondaires à une irritation chronique locale. Ils sont habituellement de petite taille, d’évolution lente et peu symptomatiques, mais certaines formes volumineuses peuvent entraîner une gêne fonctionnelle, une altération esthétique et poser un problème diagnostique avec d’autres lésions gingivales réactionnelles ou tumorales. Le diagnostic repose sur l’examen clinique, l’imagerie lorsque des facteurs dentaires ou osseux sont suspectés, puis sur la confirmation anatomopathologique. Le traitement consiste en une exérèse complète associée à la suppression des facteurs irritatifs afin de limiter le risque de récidive. Nous rapportons un cas rare de fibrome gingival géant traité avec succès sous anesthésie locale à l’Hôpital Général Idrissa Pouye de Dakar.
RÉSUMÉ
Gingival fibromas are benign connective tissue lesions, most often regarded as reactive hyperplasias secondary to chronic local irritation. They are usually small, slow-growing and minimally symptomatic, but large forms may cause functional impairment, aesthetic concern and diagnostic uncertainty with other reactive or tumoral gingival lesions. Diagnosis relies on clinical examination, imaging when dental or bone-related factors are suspected, and histopathological confirmation. Treatment consists of complete surgical excision combined with removal of local irritative factors to reduce the risk of recurrence. We report a rare case of giant gingival fibroma successfully treated under local anesthesia at Idrissa Pouye General Hospital in Dakar.
