RÉSUMÉ
L’hyperplaquettose est une circonstance de découverte rare de la leucémie myéloïde chronique, c’est une urgence diagnostique et thérapeutique du fait de son pronostic péjoratif en lien avec complications thromboemboliques. Les auteurs rapportent un cas d’hyperplaquettose majeure en Côte d’Ivoire de découverte fortuite chez une patiente d’origine asiatique. Il s’agissait d’une patiente en bon état général avec un examen clinique normal adressé au service d’hématologie clinique pour prise en charge d’une hyperplaquettose à plus de trois millions de plaquettes/mm3.Les explorations biologiques ont permis de poser le diagnostic de leucémie myéloïde chronique avec la présence du transcrit de fusion mBCR ABL de type e13a2 B2a2 avec un ratio IS BCR ABL à 53,09 pourcents.
ABSTRACT
Thrombocytosis is a rare presenting symptom of chronic myeloid leukemia. It is a diagnostic and therapeutic emergency due to its poor prognosis linked to thromboembolic complications. The authors report a case of severe thrombocytosis in Côte d'Ivoire, discovered incidentally in a patient of Asian origin. The patient was in good general condition with a normal clinical examination and was referred to the clinical hematology department for management of thrombocytosis with a platelet count exceeding three million/mm³. Laboratory investigations led to the diagnosis of chronic myeloid leukemia with the presence of the mBCR ABL fusion transcript of type e13a2 B2a2 with an IS BCR ABL ratio of 53.09 percent.
