Abstract

ABSTRACT
Pleomorphic adenoma is the most common benign salivary gland tumor, usually arising in the parotid gland. Palatal localizations are rare (approximately 10% of cases) and pose a differential diagnosis with other benign or malignant palatal tumors. We report two cases of pleomorphic adenoma of the hard palate successfully treated by surgery. A 45‑year‑old woman presented with a painless palatal swelling that had progressed over 15 years, measuring 6 × 4 cm, with no bone erosion on panoramic radiography. A 43‑year‑old man consulted for a left palatal swelling of 3 × 2 cm that had evolved over 12 months. Both tumors were excised under local anesthesia using an orange‑segment incision, with mucoperiosteal elevation and curettage of the underlying bone. Histopathological examination confirmed the diagnosis. Postoperative courses were uneventful, with no recurrence after 5 years and several months of follow‑up, respectively. These cases remind that wide local surgical excision, respecting the mucosa and curetting the bone, allows curative treatment of palatal pleomorphic adenoma with an excellent long‑term prognosis.
RÉSUMÉ
L’adénome pléomorphe est la tumeur bénigne la plus fréquente des glandes salivaires, touchant habituellement la parotide. Les localisations palatines sont rares (environ 10 % des cas) et posent un problème de diagnostic différentiel avec d’autres tumeurs bénignes ou malignes du palais. Nous rapportons deux cas d’adénome pléomorphe du palais dur opérés avec succès. Une femme de 45 ans est venue consulter pour une tuméfaction palatine indolore évoluant depuis 15 ans, mesurant 6 × 4 cm, sans érosion osseuse à la radiographie panoramique. Un homme de 43 ans a consulté pour une tuméfaction palatine gauche de 3 × 2 cm évoluant depuis 12 mois. Les deux tumeurs ont été excisées sous anesthésie locale par incision en quartier d’orange, avec décollement mucopériosté et curetage de l’os sous‑jacent. L’examen histopathologique a confirmé le diagnostic. Les suites opératoires ont été simples, sans récidive après un recul respectif de 5 ans et de plusieurs mois. Ces observations rappellent que l’exérèse chirurgicale locale large, respectant la muqueuse et curetant l’os, permet un traitement curatif de l’adénome pléomorphe palatin, avec un excellent pronostic à long terme.
