Abstract

ABSTRACT
Introduction. Posterior fossa tumors (PFTs) account for a substantial proportion of intracranial neoplasms and remain associated with significant morbidity and mortality, particularly in resource-limited settings. Data on surgical outcomes from Sub-Saharan Africa remain scarce. This study aimed to evaluate postoperative mortality, complications, and survival following surgical management of posterior fossa tumors in a tertiary referral center in Cameroon. Methodology. This descriptive retrospective study, conducted over 6.5 years at the Yaoundé General Hospital (2019–2025), analyzed the clinico-pathological and surgical data of patients operated on for posterior fossa tumors. Overall survival was estimated using the Kaplan-Meier method, and prognostic factors were identified via a Cox regression model (p < 0.05). Results. This study of 67 patients (mean age 22.7 years, 55.2% children) in Cameroon shows that posterior fossa tumors typically present with headache (88.1%), vomiting (83.6%), and obstructive hydrocephalus (85.1%). Despite resource constraints, a 92.5% gross total resection rate was achieved, primarily for astrocytomas and medulloblastomas, resulting in an 88.1% overall survival rate. Improved survival was significantly linked to total resection and adjuvant radiotherapy, while medulloblastoma was associated with poorer outcomes. Conclusion. Surgical management of posterior fossa tumors in this Sub-Saharan tertiary center achieved encouraging survival outcomes despite resource limitations. Maximizing safe tumor resection and improving access to adjuvant therapies remain key factors for improving prognosis in this setting.
RÉSUMÉ
Introduction. Les tumeurs de la fosse postérieure (TFP) représentent une part substantielle des néoplasmes intracrâniens et restent associées à une morbi-mortalité importante, particulièrement dans les milieux à ressources limitées. Les données sur les résultats chirurgicaux en Afrique subsaharienne demeurent rares. Cette étude visait à évaluer la mortalité postopératoire, les complications et la survie après prise en charge chirurgicale des TFP dans un centre de référence tertiaire au Cameroun. Méthodologie. Cette étude rétrospective descriptive, menée sur 6,5 ans à l'Hôpital Général de Yaoundé (2019-2025), a analysé les données clinico-pathologiques et chirurgicales de patients opérés pour des tumeurs de la fosse postérieure. La survie globale a été estimée par la méthode de Kaplan-Meier et les facteurs pronostiques ont été identifiés via un modèle de régression de Cox (p<0,05). Résultats. Cette étude portant sur 67 patients au Cameroun (âge moyen 22,7 ans, 55,2 % d'enfants) montre que les tumeurs de la fosse postérieure se manifestent principalement par des céphalées (88,1 %), des vomissements (83,6 %) et une hydrocéphalie obstructive (85,1 %). Malgré les contraintes de ressources, un taux de résection totale macroscopique de 92,5 % a été atteint, principalement pour des astrocytomes et des médulloblastomes, avec un taux de survie globale de 88,1 %. Une meilleure survie a été significativement liée à la résection totale et à la radiothérapie adjuvante, tandis que le médulloblastome a été associé à un pronostic moins favorable. Conclusion. La prise en charge chirurgicale des tumeurs de la fosse postérieure dans ce centre tertiaire d'Afrique subsaharienne a permis d'obtenir des résultats de survie encourageants malgré les limites de ressources. Maximiser la résection tumorale sécurisée et améliorer l'accès aux thérapies adjuvantes restent des facteurs clés pour améliorer le pronostic dans ce contexte.
