A Remarkable Case of Rhinophyma in a Black African Patient

Le rhinophyma correspond à un stade avancé de la rosacée. Il demeure exceptionnel chez les sujets à peau noire. Nous rapportons l’observation d’un homme de 60 ans, de phototype foncé, présentant depuis trois ans une tuméfaction progressive et déformante de la pointe du nez, associée à une infiltration malaire bilatérale. Les principaux diagnostics évoqués étaient un rhinophyma, un lymphangiome, un lymphome cutané et un rhinosclérome. L’histologie confirmait un rhinophyma sur rosacée granulomateuse. Bien qu’exceptionnel, notre cas souligne l’importance d’évoquer ce diagnostic devant toute tuméfaction nasale chez les sujets à peau noire.
Rhinophyma represents an advanced stage of rosacea and remains exceptional in individuals with dark skin. We report the case of a 60-year-old man with a dark phototype who had presented for three years with a progressive and deforming swelling of the nasal tip, associated with bilateral malar infiltration. The main differential diagnoses considered were rhinophyma, lymphangioma, cutaneous lymphoma, and rhinoscleroma. Histology confirmed rhinophyma arising in the context of granulomatous rosacea. Although rare, our case highlights the importance of considering this diagnosis in any nasal swelling in individuals with dark skin.
