Traumatic Knee Dislocation in Yaounde (Cameroon): Management and Functional Outcome

RÉSUMÉ
Introduction. Les luxations traumatiques du genou constituent des urgences médico-chirurgicales graves, engageant le pronostic fonctionnel du membre par la fréquence des lésions vasculo-nerveuses et ligamentaires associées. En Afrique subsaharienne, les contraintes techniques et le plateau technique orientent souvent la prise en charge vers des méthodes conservatrices dont les résultats à long terme restent peu documentés. Cette étude visait à évaluer les modalités thérapeutiques et le pronostic fonctionnel des patients adultes traités pour une luxation traumatique du genou dans trois hôpitaux de référence de Yaoundé. Méthodologie. Nous avons conduit une étude observationnelle longitudinale avec collecte rétrospective sur une période de cinq ans, de 2019 à 2024. Nous avons inclus tous les patients de 18 ans et plus présentant une luxation traumatique du genou. L’évaluation finale a reposé sur la stabilité articulaire, l’échelle visuelle analogique (EVA) pour la douleur et le score fonctionnel International Knee Documentation Committee (IKDC). Résultats. L’échantillon comprenait 17 patients, majoritairement des hommes (82,4 %), avec un âge moyen de 37,18 ans. Les accidents de la voie publique représentaient la principale étiologie (76,5 %). Les luxations postérieures étaient les plus fréquentes (42,9 %) et 88,2 % des cas associaient des lésions ligamentaires (68,8 %), osseuses (31,3 %) ou cutanées (31,1 %). Le traitement était majoritairement orthopédique par immobilisation plâtrée (64,7 %), tandis que 35,3 % des patients ont bénéficié d'un complément chirurgical, principalement par fixateur externe. Après un suivi moyen de 32 mois, la raideur articulaire constituait la complication tardive majeure (52,9 %). Le score IKDC moyen était de 65 % dans le groupe orthopédique contre 64 % dans le groupe chirurgical. Conclusion. Malgré une réduction précoce, les luxations du genou à Yaoundé sont marquées par des séquelles fonctionnelles importantes, dominées par la raideur. L’amélioration du pronostic nécessite un accès accru à la reconstruction ligamentaire précoce et une rééducation standardisée pour limiter l'impact socio-économique chez ces sujets jeunes.
ABSTRACT
Introduction. Traumatic knee dislocations are severe surgical emergencies that threaten the functional prognosis of the limb due to the high frequency of associated neurovascular and ligamentous injuries. In Sub-Saharan Africa, technical and socio-economic constraints often direct management toward conservative methods, whose long-term outcomes remain poorly documented. This study aimed to evaluate the therapeutic modalities and functional prognosis of adult patients treated for traumatic knee dislocation in three reference hospitals in Yaoundé. Methodology. We conducted a longitudinal observational study with retrospective data collection over a five-year period from 2019 to 2024. We included all patients aged 18 and older presenting with traumatic knee dislocation. The final evaluation was based on joint stability, the Visual Analogue Scale (VAS) for pain, and the International Knee Documentation Committee (IKDC) functional score. Results. The sample included 17 patients, predominantly men (82.4%), with a mean age of 37.18 years. Road traffic accidents were the primary etiology (76.5%). Posterior dislocations were the most frequent (42.9%), and 88.2% of cases involved associated ligamentous (68.8%), osseous (31.3%), or cutaneous (31.1%) injuries. Treatment was mainly orthopedic, using cast immobilization (64.7%), while 35.3% of patients received additional surgical treatment, primarily external fixation. After a mean follow-up of 32 months, joint stiffness was the major late complication (52.9%). The mean IKDC score was 65% in the orthopedic group versus 64% in the surgical group. Conclusion. Despite early reduction, knee dislocations in Yaoundé are characterized by significant functional sequelae, dominated by stiffness. Improving prognosis requires increased access to early ligament reconstruction and standardized rehabilitation to limit the socio-economic impact on these young patients.
