Erectile Dysfunction of Patients with Chronic Respiratory Disease in Mali: Epidemiological Features and Associated Factors

RÉSUMÉ
Introduction. La dysfonction érectile (DE), trouble fréquent dans les maladies chroniques, reste sous-étudiée en Afrique subsaharienne. Au Mali, aucune donnée n'existe malgré le fardeau des pathologies respiratoires. Cette étude visait à déterminer la prévalence et les facteurs associés à la DE chez les patients atteints de maladie respiratoire chronique suivis au CHU du Point G. Méthodologie. Une étude prospective descriptive et analytique a été menée d'octobre 2020 à janvier 2021 dans le service de pneumologie. Tous les patients adultes consultant pour une pathologie respiratoire chronique ont été inclus. La DE a été évaluée à l'aide de l'International Index of Erectille Function - 5, avec analyse des facteurs de risque par régression logistique. Résultats. Parmi 100 patients inclus (âge moyen 54,3 ± 9,7 ans), 59 présentaient une DE (prévalence 59%). La DE modérée était la forme la plus fréquente (39%). Une association significative a été observée avec la BPCO (p=0,003). Le tabagisme actif doublait le risque de DE (RR=2,0; p<0,001), avec une corrélation inverse entre l'intensité du tabagisme et la sévérité de la DE. L'hypertension (RR=2,1; p<0,001) et le diabète (RR=1,9; p=0,002) augmentaient également significativement le risque. Conclusion. Cette étude révèle une prévalence alarmante de DE chez les patients pneumologiques maliens, étroitement liée au tabagisme, à la BPCO et aux comorbidités métaboliques. Ces résultats plaident pour l'intégration systématique du dépistage de la DE dans la prise en charge des maladies respiratoires chroniques et pour des stratégies de prévention ciblées au Mali.
ABSTRACT
Introduction. Erectille dysfunction (ED), a common disorder in chronic diseases, remains understudied in sub-Saharan Africa. In Mali, no data exist despite the burden of respiratory pathologies. This study aimed to determine the prevalence and associated factors of ED among patients with chronic respiratory diseases at Point G University Hospital. Methodology. A prospective descriptive and analytical study was conducted from October 2020 to January 2021 in the pulmonology department. All adult patients consulting for chronic respiratory conditions were included. ED was assessed using the International Index of Erectille Function -5, with risk factor analysis via logistic regression. Results. Among 100 included patients (mean age 54.3 ± 9.7 years), 59 had ED (prevalence 59%). Moderate ED was the most frequent form (39%). A significant association was found with COPD (p=0.003). Current smoking doubled ED risk (RR=2.0; p<0.001), with an inverse correlation between smoking intensity and ED severity. Hypertension (RR=2.1; p<0.001) and diabetes (RR=1.9; p=0.002) also significantly increased risk. Conclusion. This study reveals a high prevalence of ED among Malian respiratory patients, strongly linked to smoking, COPD, and metabolic comorbidities. These findings advocate for systematic ED screening in chronic respiratory disease management and targeted prevention strategies in Mali.
