Abstract

RÉSUMÉ
Introduction. Conclusion.

ABSTRACT
Introduction. Late diagnosis of paediatric HIV in Libreville emergency departments leads to high mortality among children and adolescents. Objective. To describe the epidemiological profile and diagnostic circumstances of HIV in paediatric emergency admissions via a retrospective study at Jeanne Ebori Mother-Child University Hospital. Material and methods. All HIV+ children/adolescents hospitalised emergently between January 2019 and April 2023 (n=83) were included. Epidemiological, clinical, and outcome data were analysed. Results. Among 11,262 admissions, 83 HIV cases were identified (prevalence 0.73%), predominantly adolescents (44.6%) and orphans (49%). Only 6% received PMTCT. Diagnosis occurred during severe complications: pneumonias (39.7%), acute malnutrition (30.1%), neuro-meningeal pathologies (15.6%). Fever was the primary sign (51.8%). Profound immunodeficiency (CD4 <500/mm³) affected 19% of patients. Twenty-eight cases (33.7%) presented with AIDS at diagnosis, with 60% mortality in this subgroup. Overall mortality reached 28.9% (24/83). Conclusion. This study reveals systematically late diagnosis of paediatric HIV in Libreville, with one-third of patients at AIDS stage and 29% in-hospital mortality. These findings reflect failures in prevention and early screening programmes, demanding reinforced community-based strategies and decentralised rapid testing.
