RÉSUMÉ
Introduction. Conclusion.
ABSTRACT
Introduction. Immediate postpartum hemorrhage (PPH), the leading global cause of maternal mortality, poses a critical challenge in sub-Saharan Africa. Objective. To analyze epidemiological, therapeutic, and prognostic features of PPH at Sylvanus Olympio University Hospital (Togo). Material and methods. A retrospective cross-sectional study (July 2017-June 2022) included all PPH cases (>500ml vaginal delivery, >1000ml cesarean). Clinical, therapeutic, and outcome data were analyzed using Epi Info 7. Results. Among 73,780 deliveries, 1,092 PPH cases were identified (frequency: 1.5%). Twenty percent had received no prenatal care. Vaginal deliveries accounted for 89.7% of cases. Main etiologies were uterine atony (37.8%), uterine rupture (24.7%), perineal tears (19.2%), and retained placenta (17.6%). Initial treatment combined oxytocics (60%) and fluid resuscitation (87%). Misoprostol was used in 36.9% of cases versus only 2% for tranexamic acid. Hemostatic hysterectomy was performed in 11.5% of cases, with an overall maternal mortality rate of 6.9%. Conclusion. This alarming mortality rate reflects gaps in access to first-line treatments and calls for urgent standardization of protocols according to WHO guidelines.
