Cardiopulmonary Risks of Waste Electronic and Electrical Equipment Workers in Bamako: A Cross-Sectional Study

RÉSUMÉ
Introduction. L'exposition aux déchets d'équipements électriques et électroniques (D3E) constitue un risque émergent pour la santé cardiopulmonaire et oncologique. Cette étude visait à identifier les facteurs d'exposition associés aux maladies chroniques cardiopulmonaires chez les manipulateurs de D3E à Bamako. Matériel et Méthodes. Une étude transversale analytique de 4 mois a inclus 159 travailleurs maliens exposés aux D3E. Des analyses bivariées et multivariées (régression logistique) ont été réalisées avec SPSS 25 (seuil α=0,05), exprimant les associations par des Odds Ratios (OR) et intervalles de confiance à 95% (IC95%). Résultats. Les participants étaient majoritairement des hommes (89,0%), réparateurs (41,8%), sans éducation formelle (73,7%) et percevant <5000 FCFA/jour (76,3%). En analyse multivariée, les facteurs indépendants associés à un risque accru de pathologies cardiopulmonaires comprenaient : la participation au retrait de revêtements (OR=6,55; IC95%[2,10-20,40]), une ancienneté >10 ans dans le retrait (OR=6,55; IC95%[2,10-20,40]) ou le démantèlement (OR=4,21; IC95%[1,70-10,40]), des antécédents personnels d'hypertension artérielle (OR=4,52; IC95%[1,80-11,40]) et la consommation de tabac (OR=5,35; IC95%[1,80-15,90]). Conclusion. Cette étude identifie l'exposition professionnelle prolongée aux D3E, notamment lors du retrait et du démantèlement, l'HTA et le tabagisme comme facteurs de risque majeurs de maladies cardiopulmonaires chroniques chez les travailleurs de Bamako. Elle souligne l'urgence de programmes de protection des travailleurs, de surveillance médicale renforcée et de politiques de gestion sécurisée des D3E.
ABSTRACT
Introduction. Exposure to Waste Electrical and Electronic Equipment (WEEE) is an emerging risk factor for cardiopulmonary and oncological diseases. This study aimed to identify occupational exposure factors associated with chronic cardiopulmonary diseases among WEEE handlers in Bamako. Material and Methods. A 4-month analytical cross-sectional study included 159 Malian workers exposed to WEEE. Bivariate and multivariate analyses (logistic regression) were performed using SPSS 25 (α=0.05 threshold), with associations expressed as Odds Ratios (OR) and 95% Confidence Intervals (CI). Results. Participants were predominantly male (89.0%), repairers (41.8%), with no formal education (73.7%), and earning <5000 FCFA/day (76.3%). Multivariate analysis identified independent factors associated with increased cardiopulmonary disease risk: participation in coating removal (OR=6.55; 95%CI[2.10-20.40]), >10 years’ tenure in removal (OR=6.55; 95%CI[2.10-20.40]) or dismantling (OR=4.21; 95%CI[1.70-10.40]), personal history of hypertension (OR=4.52; 95%CI[1.80-11.40]), and tobacco use (OR=5.35; 95%CI[1.80-15.90]). Conclusion. Prolonged occupational exposure to WEEE—particularly during removal and dismantling—alongside hypertension and smoking, are major risk factors for chronic cardiopulmonary diseases among Bamako workers. These findings underscore the critical need for enhanced worker protection programs, rigorous health monitoring, and policies promoting safer WEEE management.
