Background. In sub-Saharan Africa, limited hemodialysis access drives high mortality in end-stage renal disease, with significant geographic disparities. Objective. To evaluate the clinical and prognostic impact of dialysis care decentralization at the Gagnoa center (Côte d'Ivoire). Methods. An analytical retrospective study included 180 hemodialysis patients from April 11, 2015, to December 31, 2021. Sociodemographic (age, sex, education, home-center distance), clinico-biological (etiology, hemoglobin levels, comorbidities) and therapeutic parameters (dialysis modality, anemia management) were analyzed using multivariate logistic regression. Results. The cohort (mean age 41±12.1 years; M/F ratio=1.5) had chronic kidney failure in 98% of cases, primarily of hypertensive (43%), glomerular (29%), or HIV-related (13%) origin. Severe anemia (mean hemoglobin 7.12 g/dl; range 4.8-12.1) prompted transfusions (35%), erythropoiesis-stimulating agents (12%), or injectable iron (53%). Overall mortality was 45% after a mean follow-up of 18.24 months. Four independent predictors of death were identified: lack of education (p=0.008; OR=2.5), uncontrolled HIV (p=0.009; OR=5.0), hemoglobin <8 g/dl (p=0.001; OR=2.0, and travel distance >200 km (p=0.003; OR=4.4. Conclusion. Decentralization improves geographical access but underscores the critical influence of socioeconomic and infectious determinants on survival, requiring integrated policies encompassing early screening, proactive anemia management, and optimized medical transportation.
