RESUME
Introduction.
ABSTRACT
Introduction. In Gabon, the mortality rate due to perinatal asphyxia (PNA) is 27.4%. To gain a better understanding of the situation, the objective of our study was to describe the epidemiological aspects and short-term outcomes of newborns who presented with perinatal asphyxia in Libreville. Patients and Methods. This was a prospective, longitudinal, analytical study conducted over a 16-month period from 1 January 2022 to 30 April 2023 on newborns who presented with PPA at the Jeanne Ebori Mother and Child University Hospital Centre (CHUME-FJE). The statistical significance threshold was set at P < 0.05. Results. Among the 464 newborns hospitalised, 96 presented with APN (20.7%) with a sex ratio of 1.26. The average age of the mothers was 29 years ± 7 years. Prematurity was found in 62.8% of the children. Apparent death remained after 5 minutes of resuscitation (10.5%). The average duration of resuscitation was 9.1 min ±7.1 min. The main complication was neurological (73.3%) with AIE classified as Sarnat II (27.0%) and Sarnat III (22.2%). Transfontanellar ultrasound showed cerebral oedema in 61.1% and intraventricular haemorrhage in 24.1% of children. The most common causes were obstetric (32.6%), foetal (29.0%) and maternal (22.1%) pathologies. Magnesium sulphate was used in 60.5% of patients. The average length of hospital stay was 12.6 days. The overall mortality rate was 32.6%. Conclusion. APN remains a public health problem in Libreville with high neonatal mortality rates. The best strategy should be based on prevention through better monitoring of pregnancy and childbirth and a training programme for healthcare staff in newborn resuscitation in the delivery room.
