Abstract

RESUME
Introduction : Les articulations du poignet et de la main sont les plus fréquemment touchées dans la polyarthrite rhumatoïde (PR), avec un risque évolutif vers des déformations invalidantes en l’absence de traitement chirurgical.
Objectif : Évaluer l’efficacité de la chirurgie sur la douleur, la fonction et l’évolution structurale dans la PR main-poignet.
Patients et méthodes : Une étude rétrospective descriptive (2013–2024) a inclus 9 femmes (âge moyen : 51,4 ans ; extrêmes : 38–77 ans) présentant une atteinte rhumatoïde main-poignet pour 11 membres. Les interventions réalisées comprenaient synovectomies, ténodèses, arthrodèses et procédures de stabilisation. Les paramètres analysés étaient : douleur (EVA), force de préhension, score DASH, et critères radiologiques (collapsus carpien, glissement ulnaire).
Résultats :
- Douleur : réduction significative de l’EVA moyen (8,1 à 0 au repos ; 8,4 à 0 à l’effort).
- Fonction : récupération de 137 % de la force de préhension vs côté controlatéral.
- Radiologie : amélioration du collapsus carpien chez toutes les patientes ; glissement ulnaire minime (0,36) dans 1 cas.
- Score DASH : résultats excellents (5 cas), bons (4 cas), moyens (1 cas) et mauvais (1 cas).
Conclusion : La chirurgie améliore durablement la douleur, la fonction et l’alignement anatomique dans la PR main-poignet, avec des résultats radiologiques et cliniques appréciables malgré la complexité des lésions.

ABSTRACT
Introduction: The wrist and hand joints are the most commonly affected in rheumatoid arthritis (RA), with a risk of progressing to severe deformities without surgical intervention.
Objective: To assess the efficacy of surgery on pain, function, and structural outcomes in RA-related hand-wrist involvement.
Patients and Methods: A descriptive retrospective study (2013–2024) included 9 women (mean age: 51.4 years; range: 38–77) with RA-related hand-wrist damage. Surgical procedures included synovectomies, tenodeses, arthrodeses, and stabilization techniques. Outcomes analyzed: pain (VAS), grip strength, DASH score, and radiological parameters (carpal collapse, ulnar shift).
Results:
- Pain: Significant VAS reduction (mean 8.1 to 0 at rest; 8.4 to 0 during activity).
- Function: Grip strength recovery to 137% of the contralateral side.
- Radiology: Improved carpal collapse in all patients; minimal ulnar shift (0.36) in 1 case.
- DASH score: Excellent (5 cases), good (4), fair (1), and poor (1).
Conclusion: Surgery provides sustained pain relief, functional recovery, and anatomical alignment in RA hand-wrist involvement, with promising clinical and radiological outcomes despite disease complexity.
